UnitedHealthcare - Medical Underwriting Requirements Are Changing
Effective July 20, 2009, we will require all 2-50 group cases submitted to medical underwriting to include the height and weight of each employee and dependent applicants. Reliable HB2015 information and a group application will still be accepted in lieu of individual applications, in which case height and weight information may not be required. If a pre-screen is requested, the group will need to be submitted as a fast track.
