effective dates of 2/01/2010 and 2/15/2010 not currently with our office.
Calculations are based on actual enrolling employees/subscribers at time of
enrollment. Each group is considered individually (not a cumulative total of
employees). Bonus will be paid in the form of AMEX gift check(s) approx. 60 days
after the effective date and when commissions have been paid by carrier. Group must be inforce at time of payout.
Qualifying decisions of The Insurance Exchange are final. Contest may be altered
or discontinued at any time without notice.
