UHC and PHS Agent of Record Change Procedures
AOR changes for UnitedHealthcare and PacifiCare will not be processed unless they are in the proper format.
The submitted agent of record change request must:
- Be submitted on the company’s letterhead and addressed to UnitedHealthcare/PacifiCare
- Be signed by an authorized representative of the company with printed name and title
- List the name of the new writing agent and agency to pay and what commission percent if splitting between two or more entities
- State the customer's instruction to name a new BOR supersedes other designations and terminates commissions and other payments to any prior agent
- List group coverage types and policy number
Please use the AOR Change Letter template as a
guide. Your letter does not need to follow the exact wording of the template, but it must contain the
information above in order to be processed.
